Fastur á vökudeildinni!
[After a good weekend with very few “desats”, the doctors finally decided that Bjarki was stable enough to graduate from the “Neonatal Intensive Care Unit (NICU)” and move down to the “Intermediate Intensive Care Nursery (IICN)” to join the “feeders and growers”. Yay! 🙂 But… just as I was starting to get comfortable with the idea, I got the news that the IICN is undergoing some renovations (something to do with the wiring for a new computer system) so they’re not accepting new babies as of today!
Nobody seems to know how long the IICN will be closed for new admissions; I’ve heard everything from a couple of days to two weeks. So meanwhile, we’re stuck in the NICU, which isn’t terrible, it’s just more of the same: Alarms going off all the time, periods where parents are forbidden due to rounds and shift changes and general loudness due to endless commotion.
Anyway, at lunchtime a lady from Occupational Therapy (OT) came to see Bjarki (and me) to observe how he’s doing with the bottle. He did pretty well, but he’s still recovering from his anemia spell and not quite up to where he was about a week ago. She did warn me that the next few weeks are going to be tough because one keeps hoping for a discharge, but such hopes can be dashed by a single apnea spell which pushes everything back by maybe five days. She also predicted that he would be discharged between 40-42 weeks old, which translates to Oct. 5th-19th.
Around 4pm today Bjarki almost had an MRI scan of his head (looking for bleeding) which is routine after 36 weeks of age. Apparently somebody canceled an appointment so a slot opened for him, but then the MRI people had trouble with a patient ahead of him, so he lost his spot. I was told that getting the MRI results takes about a week, and that he’s next in line so it might happen tomorrow. (Tomorrow is also eye-exam day).
Meanwhile, (recreational) breastfeeding is going all-but abysmally. I think that in the past three days I’ve only managed to try three times (due to transfusions, sleepiness, random disruptions etc…), which resulted in one “drowning” and two “not-interested”. Now, there are five feeds during the daytime: 9am, 12pm, 3pm, 6pm and 9pm. The 9am feed falls on “rounds”, so I can’t be there, and the 3pm feed coincides with a mini shift-change. We almost never make the 3pm (or 2:45pm) because we get bogged down by Bjarki’s tendency to poop while having his diaper changed which delays everything. That leaves 12pm, 6pm and 9pm.
Now, Bjarki tends to be sleepy (or trying to poop) at lunchtime and 6pm so usually not much luck there. That leaves 9pm which is tough to accomplish since by that time I’ve already gone home, and going back gets me super-tired. Complicating things even further is the fact that I’m supposed to pump before feeding to maximize the amount of fat in what little milk he gets (5mL max so far), and to reduce the flow of milk so that he doesn’t “drown” quite as easily. Sometimes I get too cosy holding him and forget to pump in time, and then that feed is pretty much over as soon as it starts.
Also, the window of opportunity that I have is pretty short, since if I dilly-dally too long with a “not really interested”-Bjarki, the nurses get worried about the schedule. So it’s this really weird race against time, the pump, diaper changes, Bjarki’s poop status, his alertness and hunger. All I know is that I can see how terribly easy it is to get totally demoralized and just give up on the whole enterprise. The bottle is so much easier! All I can hope is that my innate stubbornness withstands all this, and that we somehow manage to figure it all out before too long.]
Helgin var það góð að læknarnir ákváðu í morgun að Bjarki væri nógu stöðugur til að hann mætti fara niður á milli-vökudeildina (IICN), sem er í raun fitunar-deild. Jibbíí! 🙂 Nema hvað að skömmu seinna fékk ég að vita að IICN-deildin hætti að taka við börnum í dag vegna framkvæmda. Mér skyldist að verið væri að setja upp kapla fyrir nýtt tölvukerfi, og að það gæti tekið nokkra daga eða jafnvel tvær vikur!
Svo við erum bara ennþá á sama stað á vökudeildinni, sem er svo sem allt í lagi. Maður er orðinn nokkuð vanur viðvörunarbjöllunum, því að vera hent út á “roundum” og við vaktaskipti og svo bara hávaðanum þarna inni því það er svo mannmargt. En ég sé samt rólegheitin á hinni deildinni í hyllingum.
Í hádeginu kom kona til að sjá hvernig Bjarka gengur að drekka úr pela. Hann stóð sig ágætlega, en ekki eins vel og fyrir viku síðan (fyrir blóðleysið). Hún varaði mig við að næstu vikur verði erfiðar því að maður sé alltaf að vonast til að fara heim, en eitt lítið kæfisvefns-kast geti þýtt fimm daga á spítalanum í viðbót. Hún spáði því að Bjarki færi heim á 40-42 viku, sem þýðir 5.-19. október.
Um fjögurleytið var næstum búið að taka hann í höfuð-segulómskoðun, sem er gerð þegar börn ná 36 vikna aldri til að athuga hvort það hefur blætt inn á heilann. Það opnaðist víst tími, en þegar til kom þá var víst eitthvað vesen með sjúklinginn á undan svo ekkert varð úr skoðuninni. Það gæti verið að hann komist að á morgun, en á morgun er líka sjónskoðunardagur.
Þess fyrir utan þá verður að viðurkennast að brjóstagjöf gengur heldur brösuglega. Á síðastliðnum þremur dögum hefur mér bara tekist að prófa að gefa honum brjóst þrisvar (vegna blóðgjafa, þreytu og annarra atvika): Í eitt skipti “drukknaði” hann, og í hin tvö skiptin þá hafði hann engan áhuga. Það eru fimm matartímar yfir daginn: 9, 12, 15, 18 og 21. Klukkan 9 byrja “rounds” og þá má ég ekki vera inni í stofunni. Klukkan 15 eru míní-vaktaskipti og þar sem tómstundagaman Bjarka Freys er kúka þegar það er verið að skipta á honum, þá fara öll plön úr skorðum og klukkan verður þrjú fyrr en varir.
Eftir standa þá 12, 18 og 21. Einhverra hluta vegna er Bjarki Freyr yfirleitt sofandi eða að engjast um að reyna að kúka klukkan 12 og 18. Þá stendur eftir 21 sem er til vandræða því þá er ég farin heim, og það að fara aftur á spítalann er ekki mjög heillandi. Til að flækja málið ennþá meira þá á ég að pumpa rétt áður en ég “gef” honum til að mjólkin sem hann fær sé sem feitust, og til að draga úr mjólkurflóðinu.
Svo má ekki gleyma að ég er að keppast við matartímann sjálfan, því ef ég tek mér of mikinn tíma, þá fara hjúkkurnar að hafa áhyggjur af því að maturinn sé á eftir áætlun. Þannig að ég stend í undarlegu kapphlaupi við tímann, pumpuna, bleiuskipti, kúka-ástand Bjarka og hvort hann sé vakandi og svangur. Það er nokkuð ljóst að það er ferlega auðvelt að gefast bara upp á þessu öllu saman og snúa sér alfarið að pelanum, en ég vona að meðfædd þrjóskan dugi til að koma þessu í gang fyrir rest.
Rúmlega 10 vikna
[Bjarki had a good day today, with almost no “desats”. I went to see him around lunchtime, but didn’t try him at the boob (for his daily drowning) since right when I was about to do so, a lab-lady came to take a blood sample and he was exhausted afterwards. Boy, did he put up a fight! Finnur and Anna biked to the hospital and I left Finnur there to give Bjarki a bottle at 3pm.
Unfortunately he’d just started when the shift-change occurred (he’d gotten delayed by a-neverending-poopy-diaper-change) and got thrown out of the room. This is the most annoying thing about the NICU, but I’m told the “interemediate ICU” doesn’t kick you out nearly as much. The lab test showed that Bjarki was ok on blood, but not great, so they decided to infuse him with more blood from the same donor as yesterday. He should be crazy pink tomorrow!]
Bjarka gekk vel í dag, hélt súrefnismagninu ágætlega svona til tilbreytingar. Ég fór til hans í hádeginu og ætlaði að prófa hann á brjóstinu (þar sem hann drukknar daglega) en varð að hætta við því að það kom kona til að taka blóðsýni. Bjarki streittist heldur betur á móti og var uppgefinn eftir slaginn svo hann fékk bara mjólk í gegnum slönguna.
Finnur og Anna komu svo hjólandi upp á spítala, og ég skildi Finn þar eftir til að gefa Bjarka pela um þrjúleytið. Það tókst ekki betur en svo að honum seinkaði því að Bjarki var svo duglegur að kúka einmitt þegar það átti að skipta á honum, svo rétt þegar hann var byrjaður að gefa pelann, þá var honum hent út því það var komið að vaktaskiptum. Mér er tjáð að á milli-vökudeildinni þá sé manni ekki hent út á þessum tíma. Blóðsýnið sýndi að blóðmagn Bjarka var betra, en ekki frábært, svo að það var ákveðið að gefa honum meira blóð í dag, til að geta notað blóð frá sama blóðgjafa. Hann ætti að vera vel bleikur á morgun! 🙂
