Andað ótt og títt
[Nothing new to report regarding Bjarki really. I went to see him this evening and he was awake, and looked annoyed with the big tube going into his mouth and down his throat. He’s still on the nasal cannula, but breathing quite rapidly. They might still put him on the ventilator if he gets too tired or doesn’t perform to their liking, but the CPAP is out for now. I’m told he’ll be on antibiotics for a week, and off of milk for at least two days if not more. They like to be veeery careful.
This morning, kinda out of the blue, I suggested to Anna that she’s big enough now to sleep in her own room, and asked if she was interested in that. She got all excited and tonight she did indeed fall asleep on the guest bed in the “toy room”. We’ll see how the night goes, and the following nights. Of course we should have booted her out of our bedroom eons ago (she usually falls asleep in our bed, but sleeps in her own little bed overnight), but the opportunity just never arose. So we’ll see how it goes. The important part is to do it now, totally unrelated to the eventual arrival (hopefully) of Bjarki.]
Ekkert nýtt að frétta af Bjarka svo sem. Ég fór til hans núna í kvöld og hann var vakandi og heldur órólegur, virtist óánægður með slönguna sem liggur í gegnum munninn ofan í maga. Hann er ennþá með nefslöngu og andar ótt og títt en súrefnismagnið í blóðinu er ágætt. Það gæti verið að hann endi á öndunarvél ef hann verður of þreyttur eða eitthvað, en nefpústið er víst út úr myndinni í augnablikinu. Hann verður með sýklalyf í æð næstu vikuna og matarlaus (fyrir utan næringu í æð) í amk tvo daga, ef ekki lengur. Þeir eru mjög varkárir með matarmál hérna…
Í morgun var ég svo að tala við Önnu Sólrúnu í dótaherberginu og sagði henni upp úr þurri að ef hún vildi, þá væri hún orðin nógu stór til að sofa í dótaherberginu. Hún tók vel í það og í kvöld sofnaði hún í stóra rúminu þar. Við sjáum hvernig gengur í nótt og svo næstu nætur. Við vorum ekkert að segja henni hvað gerist ef hún vaknar í nótt, erum eiginlega ekki búin að finna út úr því sjálf! Við hefðum auðvitað átt að koma henni út úr okkar herbergi fyrir löngu (sofnar í okkar rúmi, sefur í sínu eigin litla rúmi) en það bara gerðist ekki. En núna verður þetta að fara að gerast, og helst í sem minnstum tengslum við heimkomu Bjarka.
Anna Sólrún að lita á límmiða í morgun. Hún byrjaði á því að skrifa stafinn sinn (A) en þegar myndavélin mætti á svæðið var hún búin að krota vel og vandlega yfir skriftina og neitaði að endurtaka leikinn.
Bjarki Freyri í morgun með opin augu, nefrör og magasogs-slöngu.
“Cannula”
[Bjarki’s tummy/intestines starting having trouble last night. The nurse detected some blood in his stool and so they took an x-ray of his tummy. They found it to be a little enlarged so milk feeds were stopped as of midnight. The nurse also found a lot of air in his tummy, probably due to the air being pushed in by the CPAP system. When kids start having trouble with their digestion, CPAP air apparently starts getting stuck in the tummy. I was told that the tummy had hardened overnight and the nurse said she could feel some of the intestines.
To remedy the situation, they put in an active suction on his tummy to relieve it of air. They also toyed with putting him on a ventilator to stop blowing air down to his tummy, but decided, at least for the time being to remove the CPAP and replace it with a nasal cannula. This also blows oxygen up his nose, but not under pressure. It’s basically the next step up from the CPAP, and would have been the next step anyway had everything progressed without trouble – just not this soon.
The nurse told me this morning that the cannula is making Bjarki work a lot harder to breathe, but that he’s so far holding steady. She also felt the tummy was getting softer, and that it circumference had returned to what it was on Wednesday. We’ll see how things go today, it’s still only early morning.]
Meltingarkerfið var í uppnámi hjá Bjarka í nótt og maginn stífnaði aðeins upp. Til að minnka loftmagnið í maganum var sett upp sogrör en í staðinn fyrir að setja drenginn aftur í öndunarvél (skref aftur á bak) þá var nefpústið fjarlægt og nefrör sett upp í staðinn (eins og gamalmenni með ónýt lungu fá). Þar með er loftinu ekki ýtt niður undir þrýstingi (og lungunum ekki haldið opnum fyrir hann) heldur þarf hann að anda alveg sjálfur.
Hjúkkan sagði mér áðan að hann er að erfiða heldur mikið við að anda, en að þetta gangi hjá honum enn sem komið er. Hún sagði að þetta væri mjög svipað og þegar hann var tekinn af öndunarvélinni í annað sinn og settur á nefpústið. Þá erfiðaði hann líka mikið. Maginn er líka farinn að mýkjast og er aftur kominn niður í ummálið frá síðasta miðvikudegi. Þannig að við sjáum hvað setur. Þeir voru amk glaðir með að hafa sett sýklalyfin í gang snemma, það hefur vonandi hjálpað eitthvað.
Meltingarkerfið?

Er að fara að sofa í “alvörunni” (ekki bara á sófanum) en hringdi fyrst upp á spítala. Það gæti verið að meltingarkerfið sé að valda Bjarka vandræðum, það er amk búið að stoppa mjólkurgjöf fram í fyrramálið. Hjúkkan tók eftir óvenjulegum kúk þegar ég var þar um níuleytið í kvöld og fann blóðagnir í honum. Svo þeir tóku röntgenmynd og sáu merki um bólgu. Maginn hans var líka fullur af lofti (neföndunardótið blæs líka lofti niður í magann) og það hjálpar ekki til við meltinguna. Hjúkkan sagði að þeir gætu endað á því að setja hann á öndunarvélina aftur bara til að losna við þetta blás-í-maga dót. Annars ræktaðist ekkert úr mænuvökvanum, sem er gott, og enn sem komið er hefur ekkert skotið upp kollinum í blóðsýnunum eða þvaginu. Hann heldur því áfram á sýklalyfjum.
Við fórum annars í dagslok í “pikknikk” hjá Google. Þetta var víst sumarhátíðin, með fullt af fríum mat, tónlist, trúðum og skemmtitækjum. Við röltum um og það var ágætt.
